Blazer Boosters supports and promotes all of Northland Christian's co-curricular activities, providing volunteer opportunities and community among our members, and the opportunity to be involved with the activities, sports, fine arts, and other clubs in which our kids love to participate!

SPORTS PHYSICAL FORM
As you prepare for your student to participate in sports at NCS, please print this MSHSAA form and complete pages 1-3 to submit to the doctor's office. Your physician will need to complete and sign on page 5. Once that is completed, please scan and email the form to emontgomery@ncblazers.org so that it may be uploaded to your student's documents tab in Sycamore. Your paper copy can also be scanned to Eric Montgomery at the Welcome Center.
